Please be aware that submitting information through this website, email, or contact forms may not be fully secure and should not be used to transmit sensitive health information. Communication through this website does not establish a therapist-client relationship.

Clients receiving services from Mosaic Counseling Center will be provided with a complete Notice of Privacy Practices that explains:

  • How your health information may be used and disclosed;

  • Your rights regarding your health information;

  • Our legal duties to protect your privacy; and

  • How to file a privacy-related complaint.

We are committed to maintaining the confidentiality of your information and providing services in a manner that respects your privacy and dignity.
